TITLE OF POSITION: COMMUNITY EDUCATOR/MARKETING LIAISON

TITLE OF IMMEDIATE SUPERVISOR: ADMINISTRATOR/SALES AND MARKETING

POSITION SUMMARY:
The Community Educator/Marketing Liaison is responsible for interaction in the community and promoting company services and other duties as deemed necessary. He/She reports directly to the Administrator and indirectly to the Sales & Marketing.

REQUIREMENTS:
1. Baccalaureate degree preferred.
2. One year of experience in community relations or in a health related field preferred.
3. Experience in working with professional personnel and community representatives.
4. Must possess excellent written and oral communication/skills.
5. Respect for human dignity and patients’ rights.
6. Demonstrates ability to deal with professionals and community.

RESPONSIBILITIES:
1. Educates physicians about the services available through home health and Hospice.
2. Maintains contact with referral sources to update them on changes in Medicare/Medicaid regulations.
3. Assist agency in maintaining compliance by delivering physician orders to offices and facilitating getting signature.
4. Enhance awareness of home health services in the community by interfacing with discharge planners, hospital and physician staff.
5. Maintain committed and cooperative attitude with all job duties.
6. Be available to assist the community with wellness and prevention.
7. Discuss care plans with physician offices for patient issues which need to be addressed.

Procedures related to compliance issues:
1. No person will knowingly offer to pay or agree to accept any remuneration directly or indirectly in cash or in kind, to or from any person, firm, associate of persons, partnerships, or cooperation for securing patients.
2. No person shall advertise false, misleading, or deceptive information that is not readily subject to verification.

Community Education Plan:

1. Implements community education plan staying within established timetable.
Reviews and evaluates the analyses and plan on an established basis.
3. Perform any and all other duties and responsibilities as assigned by your supervisor.
4. Stay current with regulatory changes in the home health industry.
5. Prepare educational materials.
6. Know your company and the product lines.
7. Make 75 contact calls (physician offices) every 2 weeks.
a. Develop relationships with physician offices, discharge planners, and potential referral sources.
b. Identify decision-makers at various sites.
c. Provide educational information related to the home health industry.
d. Follow-up with referral sources at least once per month: provide outcomes.
e. Increase referral base by at least 15% each quarter.
f. Deliver and pick up 485’s as needed.
g. Document work information on the tracking log and turn it in weekly to the supervisor.
h. Discuss problem patients with the DON or Administrator for potential issues to relate to physicians.

JOB CONDITIONS:

Position is stressful in meeting deadlines. Minimal lifting of office records and printouts. Primarily, a desk job which involves sitting, standing, stooping and walking. Excessive telephone communication. Must be able to read 12 point or larger type. Must be able to hear adequately on the telephone with no more than an amplifier. Must be able to hear and speak in a manner understood by most people. Must be able to communicate both verbally and in writing.

The position requires the ability to drive extensively in a geographic area. Must be able to access prospective referral sources which will not be routinely wheelchair accessible.

EQUIPMENT OPERATION:

Utilizes computer and basic office software packages (Microsoft Word, PowerPoint, Excel), Calculator, 10 line telephone, and other related office equipment.
