TITLE OF POSITION: COMMUNITY HEALTH REGISTERED NURSE

TITLE OF IMMEDIATE SUPERVISOR: Director of Nursing

POSITION SUMMARY
Nursing care is in accordance with the client’s plan of care, and includes comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention, coordination of services, teaching and training activities, and provision of direct nursing care and other duties as deemed necessary.

RESPONSIBILITIES
Professional nursing services provided by a registered nurse include:
-Conducts the admission of the patient when necessary.
-Obtains a medical history, particularly as it relates to the present condition, from the patient and/or family member(s).
-Initial and ongoing comprehensive assessment to include OASIS, evaluation of home situation to determine what health teaching will be required and services needed.
-Develops and updates the plan of care in consultation with the physician and all clinicians who perform the comprehensive assessments of the client.
-Provides skilled nursing care as needed in accordance with plan of care.
-Providing those services and/or treatments requiring substantial and specialized nursing skill.
-Communicates the plan of care with other caregivers such as physical therapists, speech and occupational therapists, social workers, LVNs and home health aides.
-Counseling the client and family in meeting nursing and related needs.
-Assesses pain management and completes review of medications.
-Assesses need for and use of health care equipment and technology.
-Participates in the patient’s discharge planning process.
-Initiating appropriate preventive and rehabilitative nursing procedures.
-Preparing clinical and progress notes
-Coordination of services
-Referral to other services as needed; Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
-Informing the physician and other staff of changes in the client’s needs.
-Evaluating outcomes of care.
-Supervises and evaluates the Home Health Aide Care Plan in conjunction with the clinician establishing the aide care plan.
-Supervises that the aide supervisory visits are conducted every 14 days and, when necessary, performs such supervisory visit.
-Assigning home health aide to a specific patient.
-Assist with the nutrition, meal preparation and fluid intake.
-Prepares and presents patient’s record to the Clinical Record Review Committee as indicated.
-Continually strives to improve her nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading.
-Supervises that LVN supervisory visits are concluded monthly and, when necessary, performs supervisory visits. Also, supervises communication with LVN is documented twice monthly concerning client status.
-Responsible for the overall clinical record maintenance to include documentation filed within 7 days, medication profile updated, timely orders/plan of care, comprehensive assessments, including Oasis is timely, etc.

-Identifies health risks and implements risk reduction strategies for self and others according to agency policy.
1. Conducts risk assessments, as indicated, according to client’s age, diagnosis, living arrangements and agency policy.
2. Reports and records incidents according to proper procedures.
3. Refers to supervisor as needed.

-Conducts or participates in team meetings/case conferences to evaluate client’s response to care, need to alter the plan of care, or need to add or discontinue a service or need to change payer status.
-Evaluates client’s response to care and discusses progress of client with physician, supervisor and/or other caregivers.
-Uses appropriate communication strategies to affect positive client care outcomes.
-Listens to others and shares knowledge in a non-judgmental manner.
-Practices with professionalism and accountability as employee of the home health agency.
1. Adheres to Agency policies.
2. Organizes, prioritizes and provides care and other work assignments in a timely and cost-effective manner.
3. Practices fiscal responsibility through timely correct coding of employee time sheet, and OASIS document.
-Provides increasing leadership and supervision.
-Demonstrates a commitment to a healthy and safe work environment.
1. Follows established federal, state or local policies, procedures and programs relating to health and safety in the workplace.
2. Takes steps to promptly correct hazardous conditions.
3. Regularly participates in training and education in safety practices and procedures, such as annual OSHA training.
OCCASIONAL DUTIES
May be requested by Administrator/CEO to fill in for the other nurses when on vacation or sick and
other duties as deemed necessary.

REQUIREMENTS
Qualifications: Graduation from an accredited School of Nursing. Must be licensed in the State as a registered nurse.

Experience: One or more years in home health agency or in a hospital setting. Home health experience preferred.

Knowledge, Skills and Abilities: Knowledgeable of Medicare and Medicaid guidelines. Good communication skills and ability to establish good rapport with other staff members. Working knowledge of home health care and the principles and techniques of professional nursing and required documentation thereof.

Skillful in organization of work and in principles of time management; and knowledge of management process. Ability to contribute to the quality of care being rendered through constructive communication with nursing managers and nursing staff.

JOB CONDITIONS
The ability to drive extensively in a geographic area. Must be able to access clients’ homes which will not be routinely wheelchair accessible. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform and demonstrate client care. Physical activities may include, walking, sitting, stooping, standing. Maximum to minimal lifting of clients and turning clients. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds. Lifting and carrying nursing equipment. Frequent writing and telephone communication. Must be able to hear and speak in a manner understood by most people. Must be able to communicate both verbally and in writing.

EQUIPMENT OPERATIONS
Thermometer, B/P cuff, glucometer, penlight, hand washing materials.
