TITLE OF POSITION: PHC ATTENDANT
TITLE OF IMMEDIATE SUPERVISOR: PHC Field Supervisor
POSITION SUMMARY
The PHC Attendant can be a full or part time employee.
REQUIREMENTS
Completion of 9th grade. High School diploma or equivalent preferred. Successful completion of a minimum of 2 hours of training related to environmental support services.
Must have reliable means of transportation to ensure timely arrival to client’s home according to visit schedules. Must submit proof of drivers license and current auto insurance if means of transportation is an automobile.
RESPONSIBILITIES
1. Attendant must report any changing conditions or problems with client.
2. Attendant must complete service plan which may be different for every client.
3. Tasks may include: bathing, feeding, shaving. brushing teeth.brushing/combing hair, washing hands/face, applying makeup and nonprescription lotion, dressing, walking, changing diapers, assisting on/off bed pan, assisting with feminine hygiene assisting with clothing during toileting, preparing toileting supplies and equipment, transferring client from one position to another on the bed or chair, assisting change with putting on/moving leg braces and prosthesis, assisting wheelchair, cleaning living area, cleaning stove Counter, refrigerator, dusting, carrying our trash, washing and driving clients clothes, changing bed linens and making bed cooking and washing client’s dishes, preparing grocery or household item shopping list, going to store to buy and pick up grocery items or medications, putting away food, accompanying client to the doctor/clinic, reminding client to take their medications.
JOB CONDITIONS
The position requires the ability to travel extensively in a geographic area. If traveling is by automobile, a copy of driver’s license must be supplied. Must be able to access clients’ homes which will not be routinely wheelchair accessible. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform and demonstrate client care. There must be the ability to give and receive information orally. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30
