Director of Services

TITLE OF POSITION: DIRECTOR OF SERVICES

TITLE OF IMMEDIATE SUPERVISOR: CEO/CHIEF OPERATIONS OFFICER

JOB SUMMARY:

The Director of Services is a qualified full-time employee who reports to CEO/COO and Board of Directors. Responsible for the overall agency’s ongoing functions including organizing and supervision of the professional, paraprofessional and office activities of the sub-unit (agency) and other duties as deemed necessary.

QUALIFICATIONS:
• Bachelor’s degree of science in Nursing is preferred from an accredited university or equivalent years of experience.
• One year of supervisory or administrative experience in home health care or related health programs.
• A licensed registered nurse in the State of Texas and one home health nursing experience.
• Experience with CHAPS accreditation, preferred.
• Leadership skills such as ability to take charge, set objectives, results oriented and team player.
• Ability to demonstrate self-confidence and positive attitude toward self and others.
• Mantains commitment and enthusiasm towards goal achievement.
• Ability to manage multiple tasks simultaneously under dealine pressure.
• Attempts the development and implementation of better methods.
• Excellent planning, analytical, interpersonal and communication skills.

RESPONSIBILITIES:
Organizes and directs the agency’s ongoing functions:
1. Supervises total operation of the agency.
2. Oversees the development of standards and methods to measure agency activities.
3. Ensure that all required policies and agreements are carried out and ensures that required records are accurate.
4. Obtains written agreements with hospitals, long-term care facilities and community agencies as set forth in the policies of the agency.
5. Participates in the review, analysis and appraisal of the effectiveness of the agency’s program.
6. Evaluates service policies and functions, and recommends proposals for changes or study of problems, which affect the agency.
7. Evaluates the performance of the individuals in the agency in relation to establish standards and the individuals’ professional development.
8. Coordinates the timely implementation of corrective action plans and controls.
9. Employs qualified personnel and ensures adequate staff education and evaluations.
10. Knowledgeable of federal, state and local rules and regulations and maintain them in the Agency.
11. Ensures the agency employees be familiar with the rules and regulations, and shall have copies of the rules available for their use.
12. Be responsible for the completion, keeping and submission of such reports and records as required.
13. Responsible for the timely submission of payroll documentation, including Paid Time Off as required by the corporate office.
14. Designate a professional employee to be his/her authorized representative in his/her absence.
15. Maintain a current organizational chart to show lines of authority to the client level.
16. Clearly identify and make public services provided by the branch and the geographic area in which these services are available.
17. Maintain an office facility for the agency which is large enough for efficient staff work, adequately equipped, and which provides for a safe working environment, meeting local ordinances and fire regulations.
18. Ensures the accuracy of public information materials and activities.
19. Develops cooperative relationship with other agencies, and with outside Agencies for exchange of information and services, and with community agencies to develop understanding of Agency program.
20. Is self-reliant and plans appropriately.
21. Shows interest, asks questions and seeks information regarding organizational opportunities within the region.
22. Responsible for all the clinical activities of the agency in compliance with federal and state regulations.
23. Oversees all CQI and utilization review activities.
24. Ensures that all required policies, procedures and agreements with hospitals, long term care facilities and community agencies coincides with the policies of the agency.
25. Oversees the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.
26. Consults plans and works with the CEO/COO in policy formulation and implementation including assessing adequate and qualified staffing.

27. Consults with physicians in matters relating to patient care services.

28. Consults and plans with health care facilities regarding staffing needs.

29. Coordinates with continuous Quality Improvement Committee. Implements corrective action plans and controls timely.

30. Confers with supervisors and staff, individually or in groups, regarding patients or families, special programs or service programs.

31. Develops standards that ensure safe and effective services to clients and families to include the following:

a. Assure a reassessment of a client’s needs is performed by the appropriate health care professional when:
i. There is a significant health status change in the client’s condition.
ii. At the Physician’s request.
iii. After hospitalization.

32. Responsible for a complete staff orientation and opportunities for continuing educational opportunities.
33. Participates in state and national organizations, meetings, seminars, workshops and activities relating to health professions and health care services, when appropriate.
34. Actively develops and pursues referral sources, as well as facilities on-going relationships with various medical and health communities.
35. Is available at all times during the operating hours of the agency or have qualified alternate in writing.
36. Promotes the organization’s mission and goals with staff and community.
37. Keeps current on local area issues and works with organizations that work to improve the health and welfare of the community.
38. Establishes and maintains effective channels of communication.
39. Attends outside meetings and/or conferences as deemed necessary.
40. Ensures confidentiality and reliability of agency’s data, records, proprietary information and intellectual property.
41. Demonstrates flexibility and willingness to work on other duties as assigned.

JOB CONDITIONS:

Position is stressful in managing the following: Meeting deadlines, multi-tasking management, delivering immediate responses, strategic planning and implementation. Minimal lifting of office records and printouts. Primarily a desk job which involves sitting, standing stooping and walking. Excessive telephone communication. Requires traveling by car or airplane to local, out of town or state seminars, conferences or meetings. Must have excellent written and communication skills to be understood by all levels of the organization and clients.

EQUIPMENT OPERATION:

Utilizes PC, calculator, multi line telephone and other related office equipment.

COMPANY INFORMATION:

Access to all personnel records and company financial records, which may be discussed with all management staff, including the Board of Directors.

PROTECTED HEALTH INFORMATION:

All the protected health information of patients needed to perform the duties of the job description. This information could be discussed with the management staff including the Board of Directors, in compliance with the information management and confidentiality policies.
